As we bring individuals closer to their community, we build relationships with our MCO, and we see changes in how our caregivers approach their work: they begin to think differently, to have more purpose, and more pride in their work.

    The MCO, Community Care Connections of Wisconsin, supports the CSL model. Working together, we believe we will make a difference, increase options, increase choices, and increase the cost-effectiveness of supporting individuals to live more fully as part of their community.

  3. Outcomes

    The identified outcomes for our CSL Journey are: 1) to create a partnership with the local managed care organization, 2) to research and develop training materials, 3) to successfully transition members into the Community Supportive Living program, 4) to explore housing options, and 5) to provide financially sustainable CSL support.
